When should I fill out an Incident Report?
The purpose of an Incident Report Form is to gather information on events, interactions, and issues of concern related to safety, security, student or employee behavior, and health. There are many situations in which it’s a good idea to fill out an Incident Report Form. They may involve the following:
- Non-Emergency Accident
- Emergency (Police/Ambulance called)
- Safety Incident
- Vandalism
- Maintenance
- Student Conduct
- Faculty Concern
- Staff Concern
- Emotional Concern
- Health Concern
After receiving a submitted Incident Report, you can expect to hear back from the Student Life and Advising Office within 1-2 business days for support, guidance, and follow-up.
If your situation is urgent (non-emergency) but you need support immediately, we encourage you to contact the SIT 24-Hour Counselling Helpline at 6592 2030.
In some situations, after verbally reporting a campus-related incident to DigiPen (Singapore) staff, you may be asked to fill out and submit an Incident Report Form. In certain cases, DigiPen (Singapore) may also communicate incidents submitted through the Incident Report Form to SIT or the police.
